Situation

Early readmission of previously hospitalized patients is a huge problem for the U.S. healthcare system: a 2009 article in The New England Journal of Medicine reported that 20% of the almost 12 million Medicare beneficiaries who had been discharged from a hospital were re-hospitalized within 30 days. The associated Medicare cost of these unplanned rehospitalizations in 2004 was $17.4 billion. The third leading medical cause of 30-day hospital readmissions in Medicare patients is Chronic Obstructive Pulmonary Disease (COPD).
